Health Info:

Shoreline Health Neighborhood

Founded in 2012 by VNA Community Healthcare, the Health Neighborhood is a group of professionals from medical offices, health departments, senior housing, geriatric care management, skilled nursing facilities and other nonprofit agencies from throughout the greater New Haven area.

The group formed as a grassroots response to healthcare reform, the changing health delivery system, and the impact these changes have on our shared patients and community. Their goal is to work together as a health community to reduce hospital readmissions and help people stay healthy and independent through professional collaboration and community health literacy education.

The Health Neighborhood's main focus is improving consumers' health literacy. They have also tackled the issue of overusing antibiotics and will work to improve coordinated care between medical facilities and community organizations.
