Physical Exams Required of Students

Students entering kindergarten and grades 3, 6, and 10 are required to have a physical examination, meet proof of immunity requirements, and must submit a completed blue physical form. Students in grades 3, 6, and 10 have the entire school year to return their completed exam report. The completed exam report for kindergarten students is to be returned to the school nurse before Tuesday, Sept. 8. Kindergarten physicals performed 12 to 15 months prior to entry into kindergarten are acceptable, provided all mandated information is complete and submitted on the State of Connecticut Health Assessment Record form.

Students participating in Adams and Guilford High School athletics must have a completed physical form on file. The revised, three page, state blue form, (4/11 or 4/12) may be used to meet the requirement for athletes. For Adams, upon selection to a team, the student must have a current physical examination form filed with the school nurse before the first competition in order to continue participation. For Guilford High School, a current physical must be on file before the first tryout or practice. The examination expires 13 months from the date of the examination. When the physical examination expires, the student-athlete is athletically ineligible to practice and/or compete. All freshmen, new high school athletes, and juniors in some sports must also take ImPACT.

For more information regarding physical examination/athletic requirements, contact your respective school nurse.
